If this is your child’s first visit with Pathway, click the link below! We must have their previous records before scheduling a wellness visit or any type of behavioral/mental health consultation. Complete the forms and return them to us via email, fax, or in person.

If your child is sick, call our office to schedule a new patient sick appointment. Your child can be seen for a sick visit before we receive records.
